Electronic complaint form

We recommend that you use our electronic complaint form. This ensures that we have the information we need to process the case as quickly as possible. The form is simple to use:

• Open the complaint form below
  The form requires Adobe Acrobat Reader 8.1.2, or a newer version. If Adobe Acrobat Reader is not installed, you can download the programme free from Adobe's website
• Select a language
  You can select your preferred language for filling in the form. It would help us if you fill in the form in English where possible (you can still select Romanian as the preferred language for the form even though you fill in the boxes in English)
• Fill in and send the form
  Fill in the form as accurately as possible and attach all the relevant documents. When you have double checked that all the information is correct, send it direct to our case processing system by pressing the "Submit complaint” button on page 1 of the form
• The reference number confirms receipt. When a reference number appears on the screen, you will know your complaint has been registered

• Keep a copy of the complaint. You can save a copy of your complaint by using the "file/save as" function in Adobe Acrobat Reader.
